The Student Award Request form from the Financial Aid office is now a DocuSign process. Please follow the steps to submit a student award form for review.
Step One:

Follow the link and scroll down to “Information for DU Staff and Faculty- Student Award Policy”. Click the link for Student Award Request Form.
Step Two:
In the Staff/Faculty Requestor- fill in your name and email
In the Budget Officer section- Name “BODR” and email support@dusaie.freshdesk.com
In Financial Aid- leave as is
Click "Begin Signing"
Step Three:
Input the student information into the form.
IMPORTANT- in the “please describe” section put in the name of the grant (e.g. Undergraduate Student Engagement Grant, Student Assistance Fund, etc.) so we can match the form to the letters. The BODR team receives this form from multiple offices so correct information is necessary for proper processing.
Step Four: Sign the form and submit. You are finished.
